Utilization of labeled syringes when administering vaccines in community/ambulatory care

The Institute for Safe Medication Practices (ISMP) continues to receive reports of vaccine errors involving vaccine and syringe mix-ups. A common contributing factor in these events is the use of unlabeled syringes when preparing and administering vaccines from vials (i.e., not in manufacturer prefilled syringes). Thus, ISMP recommends practitioners and immunizers clearly label all individual syringes containing vaccines. We are interested to learn if your practice site affixes labels to practitioner-prepared syringes for vaccines. This short five-question survey will be open until July 29, 2022. Survey responses are anonymous.
