The proposed statements will be provided on the following pages. These statements were selected for the guidelines as they represent risks identified through the ISMP Medication Errors Reporting Program (MERP), literature review, consensus comments from participants attending ISMP’s National Perioperative Summit for Medication Safety, clinical observations, and/or those best practices that demonstrated a lower national adoption rate as evidenced by recent results from ISMP’s Medication Safety Self Assessment® for Perioperative Settings. These select statements are organized by ISMP’s Key Elements of the Medication Use System™ and associated Core Characteristics.
